Our Partner: The Health Initiative
The Health Initiative was created out of a collaboration by Canton-Potsdam Hospital and the Claxton-Hepburn Medical Center. Leaders from these institutions brought together over 200 individuals—representing a broad spectrum of the community, including health, human service, business, government, and education—to conduct a thorough assessment of health in St. Lawrence County. The steering committee identified three health priority areas that needed to be addressed county-wide: access to care; nutrition and physical activity; and substance use, focusing on youth tobacco use. The Health Initiative was incorporated in 1999 as a free-standing 501(c)(3) nonprofit organization to acquire resources and programs to address the identified priorities.
The Health Initiative continues to build on this rich history of collaboration by maintaining multi-agency collaboration as a requirement of every project. The agency is also committed to supporting other community agencies in their efforts including helping others to obtain resources, sharing data, information, and materials, and through joint projects. This philosophy is summed up by the agency's mission statement: To measurably improve the health of county residents through a collaborative community effort.

The Alcoa Foundation is one of the largest corporate foundations in the United States, with assets of approximately $436 million. Founded more than fifty years ago, Alcoa Foundation has invested more than $530 million since 1952. In 2010, Alcoa and Alcoa Foundation contributed $36.8 million—$19.9 from the Foundation—to nonprofit organizations throughout the world, focusing on environment, empowerment, education, and sustainable design. Through this work, Alcoa seeks to promote environmental stewardship, prepare tomorrow’s leaders, and enable economic and social sustainability.
The work of Alcoa Foundation is further enhanced by Alcoa’s thousands of employee volunteers, who in 2010 gave more than 720,000 service hours—the equivalent of 350 people working full time. In 2010, a record 49 percent of Alcoans took part in the company’s signature Month of Service across 23 countries. Through nearly 1,000 activities, Alcoans reached 59,000 children, served 17,000 meals, planted 16,000 trees, and supported 3,000 nonprofit organizations.

The Grasseroots Fund is a donor-advised fund within the Northern New York Community Foundation. The NNY Community Foundation raises, manages, and administers an endowment and collection of funds for the benefit of the community, built and added to by gifts from individuals and organizations committed to meeting the changing needs of Northern New York.
Guided by a board of community leaders, the Foundation makes grants to support the work of nonprofit organizations and provides scholarships to individuals in Jefferson, Lewis, and St. Lawrence counties. Donors who establish funds within the foundation receive professional administration, significant tax savings, and the opportunity and satisfaction of benefiting our community now and in the future.
- The Evergreen Fund ($1,000)
The Evergreen Fund is a fund within the Adirondack Community Trust. ACT magnifies the impact of charitable giving by actively seeking opportunities where modest grants can achieve improved results. By making timely grants to select nonprofit organizations, ACT provides donors with perpetual stewardship of their charitable dollars. Through careful assessment of the needs of the Adirondack region, ACT focuses its grantmaking on several broad categories: arts and humanities, animal welfare, community development, education, environment, health, historic preservation, libraries, recreation, social services, and youth programs.
